Once you have registered for camp, your Skyline account will be activated. This account is where you can submit paperwork, view pictures while your daughter is a camp, and send her emails! If you have not already accessed your account, continue reading below.
Need help finding the health forms? Or heck, your account in general? Follow the guides below. We’ve even attached step-by-step photos at the bottom that may be of some help.
Go to Skyline’s website, www.campskyline.com. Find and click the words “Log In” at the top of our homepage. Click “Forgot password/Create password”. Enter your email address that is associated with your account and you will receive an email with instructions to get you on your way. If you have forgotten your password from last year, follow the same directions above. If you have questions about gaining access to your account, feel free to give us a call.
In your account, you will see a list of options. Find and click “Forms and Documents”. Then, find and click “Health History”. You are now in the Health History form!
Note at the top the numbers 1 thru 7 - that’s how many pages you will need fill out to complete the form. Don’t worry, you can save as you go and come back to it at your convenience. Several fields will carry over from the previous year; make sure to review and make changes if necessary. Please spend the time to be detailed. The more you include on this form, the better for all involved! Complete the health history on a computer - it doesn’t play well with tablets and smart phones.
A current physical for your camper is required before arriving at camp. You will need to take OUR form to your camper’s doctor to be signed. Already had a physical within the past year? Drop it by the doctors office or fax it and they should be happy to sign our form for you. We will not take other forms that show proof of a physical. In the “Forms and Documents” list, click and print the form. Feel free to scan and upload the signed form in the “Forms and Documents” list or mail it into us at the office.
Fun fact: iPhones have a document scanning feature built into your “Notes” app. Create a new note, find and click the plus-with-a-circle-around-it button in the bottom center of your screen. Select, “scan documents” and take a photo of each side. Click “Save” and then find the “share” icon. You can save it as a PDF and upload it to your Skyline account.
Scan a copy of each side of the insurance card that covers your camper(s). Make sure to save the file as a PDF. Go to “Forms and Documents” in your Skyline account and find “Insurance Card”. Click the upload button and upload your PDF with the images.
